Completely revised and updated, Nutrition Support for the Critically Ill Patient: A Guide to Practice, Second Edition presents an unbiased, evidence-based examination of critical nutrition across the life cycle. Taking a multidisciplinary approach, each chapter has been carefully designed to provide a comprehensive review of the literature and a detailed exploration of the practical application of this information. With chapters written by experts, you get the most pertinent and current knowledge available, bolstered by tables, figures, and case studies that make the information accessible.
New Coverage in the Second Edition:
Gut microbiota support
Short bowel syndrome
Chronic critically ill phenomenon
Professional nutrition practice guidelines and protocols
Ethical considerations
Quality and performance improvement
Many challenges remain when providing optimal nutrition to all patients under all conditions at all times. Divided into eight sections, the book covers metabolic issues, nutrients for critically ill patients, delivery of nutrition therapy, nutrition therapy throughout the life cycle, special interest groups, specific organ system failure, general systemic failures, and professional issues in the field. It keeps you informed and aware of the continuous accrual of knowledge needed to craft and provide optimal nutrition therapy for the critically ill patient.

Описание: Critically ill and critically injured patients require specialized nutrition support to avoid the complications of progressive malnutrition. There is a paucity of information providing practical solutions to these difficult clinical problems. From Nutrition Support to Pharmacologic Nutrition in the ICU focuses on the theoretical and practical aspects of the management of this high-risk patient population. Each chapter presents a state-of-the-art discussion of nutritional and metabolic issues relevant to this resource-intensive patient population and contains current references, liberal tables and figures, and the personal insights of recognized international leaders in this field.

Описание: This book explains about amino acids (AAs) which are not only building blocks of protein, but are also signaling molecules as well as regulators of gene expression and the protein phosphorylation cascade. Additionally, AAs are key precursors for syntheses of hormones and low-molecular-weight nitrogenous substances with each having enormous biological importance. For example, physiological concentrations of AA metabolites (e.g., nitric oxide, polyamines, glutathione, taurine, thyroid hormones, and serotonin) are required for cell functions. Growing evidence shows that humans and animals have dietary requirements for all proteinogenic AAs. Mammals, birds, and fish also have species- and age-dependent needs for some AA-related substances. However, elevated levels of other products (e.g., ammonia, homocysteine, H2S, and asymmetric dimethylarginine) are pathogenic factors for neurological disorders, oxidative stress, and cardiovascular disease. Thus, optimal amounts of AAs and their ratios in diets and circulation are crucial for whole-body homeostasis and health. Adequate provision of one or a mixture of functional AAs or metabolites may be beneficial for ameliorating health problems at various stages of the life cycle (e.g., fetal growth restriction, neonatal morbidity and mortality, weaning-associated intestinal dysfunction and wasting syndrome, obesity, diabetes, cardiovascular disease, the metabolic syndrome, and infertility). Dietary supplementation of these nutrients can also optimize the efficiency of metabolic transformations to enhance muscle growth, milk production, and athletic performance, while preventing excess fat deposition and reducing adiposity. Therefore, functional AAs hold great promise in improving the growth, health, and well-being of individuals.

Описание:
In the historical record there is abundant evidence that obesity was a medical and health concern as long as medicine has been practiced. The idea of diet and exercise are bulwarks in the fight against obesity in history from the time of Hippocrates to the 16th century--a span of 2,000 years. However, our scientific understanding of this problem is only a little over 200 years old. An examination of the root cause of what many consider the obesity epidemic, A Guide to Obesity and the Metabolic Syndrome traces the origins and types of obesity and its treatment.
Examining in detail the developing treatment for obesity, this book provides:
A history of obesity, including treatment, proposed causes, and perceptions
An examination of the causes and problems associated with obesity
A discussion of lifestyle, diet, exercise, and treatment strategies
A detailed look at the medications and surgeries available for obesity
The fact that we have an epidemic of obesity today that is covering the globe suggests that the strategically simple ideas of eating less and exercising more, ideas that require commitment and personal involvement by the individual, have not been very successful. As we move forward in trying to understand this problem, we need to be alert to strategies and tactics that may not require individual motivation and commitment--history has shown that they do not work well. This book supplies guidance on developing and designing novel strategic interventions against obesity and metabolic disorders.

Описание:
When an excessive proportion of the human energy requirement is derived from fat, the likelihood of obesity increases. Any such individual is at risk for diabetes and cardiovascular disease- grave and costly health hazards. The selective control of fat ingestion is a promising solution to these concerns. Existing data suggests that macronutrient intake can be manipulated. Further research is working to create pharmacological tools that will suppress fat consumption. It will also be possible to fight obesity, heart disease and diabetes. Neural and Metabolic Control of Macronutrient Intake systematically discusses the known physiological mechanisms involved in macronutrientselection, including their molecular, genetic and neurochemical aspects. The book is also a critical review of the hypothesis that ingestion of the three nutrients is regulated by separate neural control mechanisms, leaving open the possibility that strategies could be devised to intervene in bodily control systems and alter the proportion of fat in the diet. This reference provides three types of information: First, the basic background of the biochemical and physiological systems as they relate to macronutrient selection. Second, opinions and data concerning to what degree animals and humans show evidence of macronutrient selection. And, third, evidence about how the central nervous system might be involved in the choices animals make among macronutrients.
